The month and date of diagnosis is included after patient's first name and state they live in.

Application for Participation

Selected candidates will be called for a personal consultation to get to know you better before the photography session.  Those selected will be expected to provide written thoughts on their experiences to share with others as seen on this blog. If you’re willing to do so, please proceed with completing the application information listed here.
